Care Assessment, Care Planning and Implementation

Within 30 days after admission, the Director, Administrator, and Developmental Trainers will create an Individualized Service Plan (ISP) based on the resident’s needs.  This plan will be reviewed with input from Developmental Trainers every 6 months.  The ISP is reviewed at staff meetings so all staff members are aware of the resident’s specific needs, approaches, and goals.

Family members and guardians are invited to attend a Care Conference within 30 days of admission, and to every 6-month review thereafter.  Family suggestions are elicited and added to the ISP.  If the resident has a visiting nurse, he or she is also invited to the Care Conference.  The purpose of the conference is to create a plan that identifies the resident’s individual needs and to establish goals.

The Administrator and Developmental Trainers meet daily to discuss each resident’s personal care, behavioral concerns, socialization and medical needs. 

 

Therapeutic Activity Programming

 The Director, Administrator, and the Developmental Trainers, along with family input assess each resident’s interests and abilities and develop a service plan that promotes individual leisure interests.

 

Health Monitoring

Vitals and weight will be monitored once per month as necessary, or more frequently upon physician’s request.

 

Medication Distribution

The Developmental Trainers and Administrator are trained to distribute medications. The documentation of this training is a requirement of the HFS 83. 

Medications are bubble-packed through the pharmacy of choice.

 

Hygiene Services

Staff will implement a service plan defining the resident’s specific abilities and needs regarding hygiene and personal care.  Each resident will be encouraged to be as independent as possible, and the Developmental Trainers will offer assistance as needed. Staff always supervises baths or showers.

 

Nutritional Services

We offer three meals plus snacks daily.  Meals are planned and all special diets ordered by a physician are honored.  Nutritional supplements are offered when weight gain is desired.

 

Housekeeping and Laundry Services

Resident’s laundry and linens are washed separately, either by the residents themselves or by the staff.  Staff cleans the resident’s bath area daily.  The Developmental Trainers provide daily monitoring of bedrooms, and cleaning assistance is provided as needed.

  

Transport Services

We will provide transportation to the resident’s medical appointments.

 

Money Management Services

Upon written request of the resident or their appointed Financial Power of Attorney, Advocate Homes will make available a separate non-interest account for use by each resident.  The account will hold up to $200.00 for a resident’s personal items, haircuts, restaurants, and movie outings. The Administrator will maintain the account, and funds will be available Monday through Friday from 8:00 a.m. to 4:00 p.m.  Residents should plan ahead for evening and weekend outings.  Each resident will receive a monthly statement.

Day Programming

We have an on site day program to meet our residents’ need for further educational and prevocational skills in a small-group setting if they choose not to attend a local workshop setting.

 

CBRF Training 

Advocate Homes provides state-approved CBRF training to anyone in need of State of Wisconsin CBRF certification. For more information on classroom dates and times, please call 262-652-3536.
